Adpem 500mg Injection Pemetrexed is an antifolate chemotherapeutic agent used primarily in the treatment of non-small cell lung cancer and malignant pleural mesothelioma.

Composition: Pemetrexed 500mg

Mechanism of Action:

Pemetrexed is an antifolate chemotherapeutic medication that functions by blocking important enzymes such as GAR Transformylase, Dihydrofolate Reductase, and Thymidylate Synthase that are involved in DNA synthesis and folate metabolism. It interferes with the synthesis of vital elements needed for the proliferation of cancer cells by preventing the synthesis of purines and pyrimidines. Rapidly dividing cancer cells are prevented from replicating by this inhibition, which ultimately results in cell cycle arrest and cell death.

Contraindications/Warnings:  A critical and clinically essential requirement for pemetrexed, which was entirely missing from this entry, is mandatory premedication: patients must receive vitamin B12 injections and daily oral folic acid supplementation starting before the first dose and continuing throughout treatment, as this substantially reduces the risk of severe hematologic and gastrointestinal toxicity. A corticosteroid (such as dexamethasone) is also typically given the day before, of, and after each dose to reduce the risk of skin rash. Pemetrexed is primarily eliminated renally and is generally not recommended in patients with significant renal impairment (creatinine clearance below 45 mL/min). Concomitant use of NSAIDs, including aspirin, should be undertaken with caution around the time of dosing, particularly in patients with renal impairment, since these drugs can reduce pemetrexed clearance and increase toxicity. Myelosuppression is common and dose-limiting. Severe cutaneous reactions have been reported. Contraindicated in patients with known hypersensitivity to pemetrexed.
