Duovir N Tablet

Duovir N Tablet is a fixed-dose combination antiretroviral medication used in the treatment of Human Immunodeficiency Virus (HIV) infection. It has strong antiviral effects by combining two nucleoside reverse transcriptase inhibitors (NRTIs), Lamivudine and Zidovudine, with a non-nucleoside reverse transcriptase inhibitor (NNRTI), Nevirapine.

Composition: Lamivudine 150mg, Zidovudine 300mg, Nevirapin 200mg 

Mechanism of Action:

Nucleoside reverse transcriptase inhibitors (NRTIs), such as zidovudine and lamivudine, function by inhibiting HIV reverse transcriptase through nucleoside analogs. During replication, they are integrated into the viral DNA chain, resulting in the chain's early termination. By directly binding to reverse transcriptase and blocking its RNA- and DNA-dependent polymerase activities, the non-nucleoside reverse transcriptase inhibitor (NNRTI) nevirapine efficiently stops the virus from replicating.

Dosage and Administration:

The dosage is to take one pill twice a day. This omits the recommended 14-day lead-in dosing period at half the maintenance dose (once daily) with nevirapine-containing regimens, used to reduce the risk of severe rash see Contraindications/Warnings above. This should be confirmed with the prescribing physician. The administration method is oral. suitable for both food and non-food consumption.

Contraindications/Warnings: 

This combination carries several serious boxed-warning-level risks. Nevirapine carries a boxed warning for severe, life-threatening, and in some cases fatal hepatotoxicity, and for severe, life-threatening skin reactions including Stevens-Johnson syndrome and toxic epidermal necrolysis these risks are highest in the first 18 weeks of treatment, and women with higher CD4 counts (>250 cells/mm³) are at particularly increased risk. Because of this, nevirapine-containing regimens are typically started with a 14-day lead-in period at half the maintenance dose to reduce the risk of rash, a detail that was missing from the Dosage and Administration section below and should be confirmed with the prescriber before starting. Zidovudine carries warnings for hematologic toxicity (anemia, neutropenia see Side Effects below) and, with prolonged use, myopathy. As with other NRTIs, lactic acidosis and severe hepatomegaly with steatosis, sometimes fatal, have been reported. Contraindicated in patients with known hypersensitivity to any of the three components, and nevirapine is contraindicated in patients with moderate to severe hepatic impairment.
